From the Parents/Guardian

• Student's birth certificate
• Student's social security card
• Student's health/shot record
• Legal documents as needed
• Proof of residency/military orders

School Information

• Addresses, phone numbers, other contact information
• Course description/grading scale
• Copy of the cover of each textbook or the ttitle page
• School handbook
• School web page address

School Records

• Copy of cumulative folder ( only the copy mailed is considered official)
• Current schedule
• Report card
• Withdrawl grade of progress report
• Test scores ( standardized tests)

Special Programs Records as Appropriate

• Individual Education Plan (IEP) Individual Accomodation Plan (504) or Gifted Program Description
• English as a Second Language or Bilingual Education
• At-risk or othe action plans for classroom modifications
