To enroll your student in Ozark City Schools, you must take the following to the school office:
- Birth Certificate
- Social Security Card
- An up-to-date original Alabama Immunization Card (blue shot record)
- Proof of Residence within Ozark City limits (Electricity bill)
- Legal Guardianship verification (may be required)
- Last Report Card showing grade placement and/or Unofficial Copy of Transcript and/or Withdrawal Papers from previous school
Under the following circumstances, you will need to contact Mrs. Sheila McLeod at the Ozark City Board of Education office:
- Student resides with someone other than parents or legal guardian
- Student and parents or legal guardian reside in another household
- Student resides outside of the Ozark City limits and seeks enrollment in Ozark City Schools - Non Resident Student Policy

The enrollment of homeless, migrant, and limited English proficient children and youth
shall not be denied due to lack of the above information. Every effort will be made
to assist families in obtaining required documents.
