Registering a Child
Items Needed for Registration:
1. Proof of Residence-Utility bill from Alabama Power/Pea River and/or water bill in your name from city utilities dept.
If you live with someone, you must take the property owner or lease holder with you to the Board of Education on East Andrews Ave. You must co-sign an affidavit in the office of Sheila McLeod stating that you live with them .
Directions to the Board of Education:
Turn left from the Lisenby driveway and go to 4-way stop.
Turn right.
Go to flashing caution light after railroad track.
Turn left.
At 1st stop light, turn right (courthouse on corner)
Go straight through – 2nd stoplight (CVS on right)
3rd stoplight (Wachovia on left)
4th stoplight (Baptist church on right)
5th stoplight
Past 5th stoplight (approx. ½ mile down road) on right you will see building facing East Andrews Avenue (just past cemetery). This is Ozark Board of Education. Ask for Mrs. Sheila McLeod.
2. Alabama Immunization Form
Directions to the Dale County Department of Health:
Turn left from the Lisenby driveway and go to 4-way stop.
Turn right onto Roy Parker Road. Go to the caution light.
Continue going striaght through the caution light.
The Health Department is on the right approximately 1 mile after you go through the caution light.
3. Birth Certificate-The school will copy the original and return it to you.
